Notice of Privacy Practices
DiagnoCure Oncology Laboratories and its employees, contractors, and affiliates are committed to maintaining the privacy of your protected health information (“PHI”) that is provided to us. PHI is information about you, including basic demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.
The Health Insurance Portability and Accountability Act of 1996 Privacy Rule (“HIPAA Privacy Rule”) (among other state and federal laws) regulates DiagnoCure’s use and disclosure of your PHI. The HIPAA Privacy Rule requires DiagnoCure to notify you of its legal duties and privacy practices with respect to your PHI. This Notice of Privacy Practices (“Notice”) describes to you how DiagnoCure may use and disclose your PHI and describes your rights with respect to your PHI. DiagnoCure is required by law to abide by the terms of its current Notice.
